Welcome
“It is our honor and our daily commitment to provide compassionate care to our patients and comfort to their loved ones at the end of life. Our passion is to deliver hospice care that brings peace and dignity to those with limited life expectancies through comfort care, pain management, and psychological and spiritual support one day at a time. Our goal is to be the standard by which the best possible hospice care is measured.”

Nancy Jakus, CEO
Countryside Hospice

Countryside Hospice is a Medicare, Medicaid and JCAHO accredited program providing superior end-of-life care services to our neighbors in the Southeast. Founded in 1999 to better serve the very special needs of the terminally ill, Countryside Hospice believes that quality in living can be achieved through patient-driven choices and the manner in which they wish to live out their lives.

One of Countryside Hospice's key goals is the promotion of a higher level of understanding of the Medicare hospice benefit, the levels of care and the services available. To achieve this, we provide well-trained staff to work with patients, families, physicians, hospitals, discharge planners, and nursing homes.

Countryside Hospice is especially proud of its contribution serving people residing in our rural counties in Georgia and Alabama by:

• Developing high standards of care;
• Creating and delivering educational competency programs;
• Advancing palliative care specialist programs;
• Championing definitive palliative care; and
• Exploring quality of life outcomes in the terminally ill patient.
